Dietitian 

Registered Dietitians are qualified health professionals that assess, diagnose and treat dietary and nutritional problems at an individual and wider public health level.

Home Visiting Team

The Home Visiting team consists of Nurse Practitioners and Paramedics. The team support all the practices by conducting home visits to patients who are too unwell to leave their home or live within a Care Home.

Medicine Management

The Medicines Management team is made up of practice and PCN staff. They all assist with the issuing of prescriptions for medication and the monitoring and review of patients that take medication regularly.

Social Prescribers

Social prescribing is a key component of Universal Personalised Care. It is an approach that connects people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing.

Mental Health Nurse

Mental health practitioners contribute to the NHS Long Term Plan ambition to develop new and integrated models of primary and community mental health care, to support adults and older adults with severe mental illnesses to live well in their communities

Physio

The vast majority of musculoskeletal first contact practitioners are physiotherapists with enhanced skills. They can help patients with musculoskeletal issues such as back, neck and joint pain
